This online form is intended for managers and employees to request the creation or renewal of affiliate accounts  for non-employees, visitors and spouse/partners.

The Human Resources Department will use this information to have the system create accounts for the affiliate, including email, netware, Glow, on-line directory listing and wireless access, as appropriate to the affiliation type.
