Affordable Care Act, Form 1095-C

One of the requirements of the Affordable Care Act is for employers and insurance companies to provide proof of health coverage. This is required to prove employees were covered in 2016 and, therefore, not subject to penalties.

Form 1095-B, Health Coverage

Many of you may have already received Form 1095-B from Blue Cross Blue Shield of Massachusetts if you were covered under Williams health plan in 2016. This form serves as proof of minimum essential coverage (MEC) for the 2016 tax year.

1095-C, Employer-Provided Health Insurance Offer and Coverage

Form 1095-C has been sent to employees who were full-time in 2016. These forms provide proof of offered coverage should the IRS request it. Please retain these forms with your tax records.
