To view and download a copy of your form, please login to https://bswift.williams.edu. Scroll down to the ‘My Forms’ section on the left side menu of the homepage and click on ‘Form 1095-C’ to download a PDF copy.
You do not need this form to file your personal tax return, you should save a copy with your tax records.
