This online form is intended for managers and employees to request the creation or renewal of affiliate accounts for non-employees, visitors and spouse/partners.
The Human Resources Department will use this information to create accounts for the affiliate. Affiliate access will be determined by the affiliation type, as indicated when you make the selection. Please request special access (including online directory listing) in the comments section if appropriate.
Please note: Spouse/Partners of Faculty and Staff will automatically be eligible for an ID Card (and Library privileges) without an Affiliation Request, as long as they are set up as a dependent or beneficiary in PeopleSoft by the Benefits staff.
